What is chronic condition management?
It is ongoing medical care for long-term conditions that need regular attention. That can include follow-up visits, medication review, lab monitoring, care planning, and coordination with other providers.
Which conditions do you manage?
High blood pressure, type 2 diabetes, high cholesterol, thyroid disorders, asthma, COPD, arthritis, chronic joint pain, and patients managing more than one condition at the same time.
Do I need a referral?
Often, no referral is required. Some insurance and Medicare plans may request one from your existing provider. Our team can verify your coverage and help coordinate any needed referrals.
How often will I be seen?
It depends on how your condition is trending. Follow-ups are typically every 1 to 3 months, and sooner when something needs closer attention.
Can I manage my condition by telemedicine?
Yes - follow-ups, medication management, and lab reviews can be handled by telemedicine throughout Florida. Some visits require an in-person exam; your provider will let you know.
Do you accept Medicare and major insurance?
We accept most major insurance plans, including Medicare. Call (813) 443-5370 to verify your specific coverage before your visit.
How long are appointments?
Initial visits are typically up to 60 minutes; follow-ups are 30 to 45 minutes. The extra time lets your provider review your results and adjust your plan with you.
Do I need a membership?
No. You can pay per visit, or join the Monthly Medical Membership ($99 to start, then $39/month) for extended visits and direct provider messaging.
Can you coordinate with my specialists or rehab?
Yes. Because we share one record across primary care and rehabilitation, your care team can coordinate treatment plans, referrals, and progress.
